Give each entry a clear purpose
Use a table with date, organization, contact or reference, channel, summary, documents, and next action. Keep separate claim numbers distinguishable. A property adjuster and an injury adjuster may be handling different questions even when the event is the same.
The California insurance department’s accident guide provides official context for claim communications. Check the rules and complaint resources of your own state when a problem arises rather than assuming one state’s process applies everywhere.
Write a factual summary promptly
After a call, note what was asked and what was actually said. Use “the representative said they would review the estimate” rather than “the estimate was approved” unless approval was clearly communicated. Save the written confirmation if one arrives.
An invented entry might read: “September 12, adjuster requested the revised invoice; sent DOC-018 through the portal; receipt confirmation saved; no payment decision yet.” This is more useful than “called insurer again.”
Track documents as well as conversations
Record the exact version of an attachment, how it was delivered, and any confirmation. If you correct a document, identify the correction rather than sending a different version with the same ambiguous filename. Keep sensitive records in the secure channel designated for the claim.
Do not record calls without checking the applicable law and permission requirements. A written personal note can still document your understanding, but label it accurately and preserve uncertainty where you are unsure.
Escalate a defined issue
If an issue remains unresolved, summarize the specific request, dates, documents provided, and response sought. Ask for the appropriate supervisor or review process. The California automobile mediation page describes one specialized dispute route with eligibility limits; it should not be assumed available for every claim.
Keep legal deadlines on a separate verified calendar. Repeated follow-ups do not necessarily preserve a claim or extend a filing date. If an important deadline, coverage dispute, or release is involved, seek qualified legal advice. A communication log helps explain the history accurately; it does not turn informal discussions into an enforceable agreement.
